2/20/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client upon arrival to the home. QMHP-C arrived to the home to conduct an individual session with Client. QMHP-C informed Client that the purpose of the session was to focus on Client demonstrating effective communication skills. Mother reports client will shut down for about 10-20 minutes and fail to communicate with is mother and father his feelings when he cannot have his way. 2. Behavior: Client appeared to be receptive to the interventions that were provided to him as well as him presenting cooperative behaviors throughout the session; however, at times he would get upset with QMHP-C as well as his mother when discussing his behaviors and how to manage them within the home setting as behaviors last about 10 minutes in duration. 3. Behavior: During the third portion of the session, QMHP-C discussed emotional conflicts to assist with pinpointing possible triggers. 1.Intervention: QMHP-C engaged Client in a therapeutic activity that consisted of ways to communicate. QMHP-C role-played and conducted role-reversal scenarios in which he was taught how to be assertive, communicate effectively while still being respectful in nature. 2. Intervention: QMHP-C educated Client on the importance of communicating with his peers to avoid conflict resolution in the home and community setting. QMHP-C discussed with Client that when he exhibits positive communication skills then he will get far in life when it comes to things that he wants in the community and in the home. 3. Intervention: QMHP-C engaged Client in therapeutic activity via cellular device to work toward gaining skills for anger management. QMHP-C explored Client’s triggers and fuses that causes him to have difficulty with expressing his feelings and frustrations of anger. 1.Response: Client appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. During the therapeutic activity, Client participated in the role-playing and role-reversal scenarios in which he was taught how to be assertive, communicate effectively while still being respectful in nature. Client laughed during some of the re-enactment of the scenarios. Client expressed that he was fully aware that when or if he commits disruptive behaviors, there might be consequences. 2.Response: Client verbalized that he does get mad when he does not get his way, but at times he feels as if he is justified because he is always being told that he can do something, but then someone changes their mind. Client did well with replacing negative and angry thoughts will more positive thoughts. 3.Response: Client stated that no changes needed to be made towards his safety plan. Client struggles with controlling his anger and frustrations when upset but has learned how to deal and cope with how he reacts when upset. Through the therapeutic activity, Client as able to recognize his own faults as it pertains to his anger. Client expresses that he struggles with feelings and thoughts that makes him angry.
